FEES

Fees for service are $300 for a 60- 90 minute initial evaluation, which are required by most insurances.. In this session we will discuss your therapeutic needs and determine together suitable treatment approaches to best meet these needs.  Individual sessions to follow will be charged at a rate of $275 for a 50 minute session. For those interested in paying out-of-pocket, rates may be available at a discounted rate of $165 per hour. Additional sliding scale options may be available according to financial need and must be negotiated prior to initial session or prior to next session if financial needs change. I am happy to discuss sliding scale possibilities based on financial need.

INSURANCE

I accept the following insurances: Regence Blue Cross/Blue Shield, First Choice, Moda, Pacific Source, OHP Open Card, AllCare OHP and Jackson Care Connect OHP.

I currently do not bill other insurance companies directly, but can provide you with the necessary forms to seek reimbursement from your insurance company if your plan will accommodate this. If you would like more information about how to inquire about this possibility with your insurance company we can discuss this in an initial phone consultation. 

24 HOUR CANCELLATION POLICY

If a session is cancelled less than 24 hours before scheduled time, the cost of the full session will be owed. In the case of documented illness or emergency, session may be rescheduled without additional charge.
